What Information Do We Collect? Like many Web sites, LifeScan.com collects information from its visitors, which can include children under the age of 13. In some areas of our site, such as our Feedback Form, Kids' Clubhouse Cookbook Survey and Travel/Role Model section, IN TOUCH Download, and Meter Warranty Registration, we collect personal information. We use this information in a variety of ways: to register a product, respond to a question, send a visitor a requested item, improve our visitors' experiences at the site, and so on.
We do not condition (and are prohibited by law from conditioning) a child's participation on the child's disclosing more personal information than what is reasonably necessary to participate in an online activity. Required personal information is clearly marked as such (often with an asterisk). We may sometimes also ask for optional information to help us get to know our young visitors and provide them with information we think they may find of interest (including about product offerings). Whenever we collect optional personal information, however, we tell our visitors that they are not required to provide such information.
The types of personal information we may collect at this Web site from children under age 13 include: first and last name; home address; e-mail address; phone number; age/birthdate; gender; ethnicity; date of diabetes diagnosis; testing frequency; blood glucose meter usage; diabetes medications/insulin usage; product purchase information; and product serial number.
How Do We Collect Information? We typically collect personal information at this Web site by asking for the information directly from our visitors in connection with various activities: Product Warranty Registration (for meters of software); Surveys to qualify for special promotions and product information, Requests for information or diabetes-related materials; Optional surveys to receive diabetes reference materials; and Feedback about our site, company or products.
LifeScan.com collects registration information so we can contact customers about product notifications or updates. Collecting this data also facilitates our compliance with FDA regulations. Although we retain customer records, anyone providing registration information can choose to not receive future mailings from LifeScan, or our Johnson & Johnson affiliates, by selecting the appropriate box on the registration form or by submitting a "Do Not Mail" request.
In order to participate in sweepstakes, contests, promotions, or other similar activities where the participant could receive something, we may ask for the participant's name, address, e-mail address, phone number, and meter serial number so that we can administer the activity, notify winners (as applicable), and provide the participant with the object of the activity (e.g., the prize, coupon, sample, literature, etc.). We may also ask for diabetes testing/usage information in connection with such activities, but we tell our visitors that they are not required to provide such information.
When our visitors complete registration forms or questionnaires, we ask for contact information, such as the visitor's e-mail address, as well as other information (age, state, preferences, and so on). Based on that information, we may send our visitor information that we think might be of interest from LifeScan or one of our affiliates. We will not, however, send such information if that visitor has clicked a box indicating that he or she does not want to receive such mailings from us.
In order to respond to a visitor's feedback or answer a question provided by a visitor, we ask for the visitor's contact information (e.g., e-mail address). All Feedback Form fields are optional, except for the subject line of the message (which helps us categorize the feedback so we can respond more efficiently). Although providing an e-mail message is also optional, we state on our Feedback Form that we cannot respond to a visitor's message without an e-mail message.

These ways include: responding to an e-mail question, selecting contest winners, fulfilling a requested transaction, and recordkeeping/product registration. We may sometimes analyze data that we collect in the aggregate to share with other Johnson & Johnson affiliates, business partners, and third parties regarding our Web site visitors (e.g., two-thirds of our visitors are over the age of eight; half of our registered visitors are boys). Such information can be important for a number of purposes, including research, marketing, advertising, developing Web site content for our visitors, attracting sponsors, and so on.
Do We Share Personal Information with Third Parties? LifeScan.com does not rent or sell the personal information of children under age 13 that is collected via LifeScan.com. In order to provide the product or service requested by a Web site visitor, we do share personally identifiable information with our business partners (fulfillment houses, and delivery services such as UPS or the U.S. Postal Service). Moreover, as indicated above, we sometimes share personal information with our affiliates so that they can send information to our visitors that we think could be of interest to them. Any such affiliates with whom we share such personal information have agreed to maintain the confidentiality, security, and integrity of personal information provided to them and to comply with this Children's Privacy Policy. Our young visitors (or their parents) can choose to not receive future mailings from LifeScan or its affiliates, by selecting the appropriate box on the registration form or by later submitting a "Do Not Mail" request.
Our Promise to Parents In most circumstances, we do not collect personal information directly from children under age 13. Instead, we will request that our Web site visitors under age 13 ask their parent to send the personal information being requested in connection with the particular activity. In such instances, we will typically ask that the child request the parent to send us an e-mail containing the requested information for the particular activity.
Before this Web site collects personal information from a child under age 13, we will:
(1) Provide the child's parent with notice regarding what personal information will be collected, used, or disclosed and what the parent's options are; and
(2) Obtain parental consent, except in the following circumstances:
- To respond to the child's one-time request (e.g., sending us a question via e-mail). In such instances, we will collect the child's e-mail address and then delete such contact information from our records after responding.

- To help us provide notice to the parent or obtain parental consent in the first place. We will delete the name and online contact information of the parent and/or child from our records if parental consent is not obtained within a reasonable time following collection of the information.
A parent may review his or her child's personal information and have it deleted or changed by contacting CustomerService@LifeScan.com.
A parent is always free, whether initially or later on, to grant consent to the collection and use of his or her child's personal information while at the same time prohibiting disclosure of such information (whether sharing, selling, renting, or otherwise providing to third parties, or making publicly available in identifiable form by any means).
