Guidelines & Requirements for Grants
LifeScan, Inc. is a Johnson & Johnson company. As a leader in the development of cutting edge medical devices in the field of diabetes, LifeScan is aware that keeping up with advances in technology is critical to the professional development of diabetes management. To help provide this service, LifeScan supports Educational Grant Programs while maintaining compliance with federal and state regulations, and AdvaMed, Johnson & Johnson and LifeScan guidelines.
Types of Grants LifeScan Supports
- Accredited Continuing Education: Educational programs for health care providers such as physicians, nurses, and/or pharmacists that provide continuing education credits.
- Non-Accredited Professional Educational Activities:Educational programs for health care providers such as physicians, nurses, and pharmacists that do not provide continuing education credits. As a condition for funding, to the extent LifeScan products are discussed as part of such funded activities, LifeScan requires that such discussion be done in a manner consistent with approved labeling.
- Patient Education: Printed/online educational materials and/or live educational events produced for people living with a particular disease or condition.
- Fellowships:Support for bona fide fellowship programs at teaching institutions, medical societies and cooperative groups. We may also support travel scholarships for fellows, residents or health care providers in training to attend relevant major medical or scientific conferences for the purpose of education. All support is in agreement with the AMA Council on Ethical and Judicial Affairs Gifts to Physicians from Industry.

Exclusions Educational grants cannot be used to support:
- Programs that have already occurred.
- Food and beverage expenses at non-accredited programs.
- Promotional activities related to LifeScan’s products.
- Development of treatment guidelines.
- Normal organizational overhead, staff training, or provision of medical care.
- Gifts and/or incentives for attendees of non-accredited educational programs.
- Travel, lodging, or honorarium for a healthcare professional to present a paper or poster.
- Educational activities for which attendee (patient or healthcare professional) travel and/or lodging is paid by the meeting organizers. This includes attendee scholarships and travel vouchers.
- Marketing sponsorships or exhibit opportunities.
- Unrestricted grants.
- Debarred/excluded institutions.
- Grants given to an individual or a private physician clinic.

General Requirements For All Educational Grant Requests Signed Letter of Request (LOR) on organizational letterhead which includes:
- Statement of educational need for the proposed activity
- Program overview, date and location
- Accreditation information (if applicable)
- Program agenda and/or topics and faculty (confirmed or proposed, if applicable)
- Funding amount and utilization of requested funds
- Complete contact information including name of requester (individual and organization), federal tax identification number, shipping address (not P.O. Box), telephone, fax, and e-mail address affiliated with requesting organization (not personal e-mail)
Form Required – please ensure that each grant submission includes the Budget Template Form found on the Web site as revisions may be made at any time.
